NOTICE TO PROXY HOLDER: Any proxy holder must present the signed proxy to the DCSA representative checking in voting members prior to the commencement of the AGM at the appropriate table and must sign in for the voting member whose proxy he or she is holding. DCSA will retain the signed proxy. The proxy holder will then receive a ballot.
